What is Case Management?

Case Management is a service that coordinates community services to help frail older adults stay safely in their own homes. Case Management will not take the place of family, friends or neighbors who are already helping you. It will support their efforts and fill in where needed. To qualify, clients must be at least 60 years old, and need help coordinating in-home services.

How Does it Work?

In order to understand your needs, a visit will be arranged in your home. You will be asked questions to assess your situation and determine which services meet your needs. You choose the services you want and your Case Manager will help arrange and coordinate the services. Your Case Manager will continue to be involved, checking in with you to assure that you continue to receive the assistance you need.

What Does it Cost?

There may be no cost to participate in the Case Management service. There may be fees for the services that you choose.
