• Billing for services or supplies not provided.

• The use of another person’s Medicare or Medicaid card to obtain medical care.
• Claim for services that are not medically necessary.
• Excessive charges for services or supplies.
• Billing Medicare a higher rate than non-Medicare patients.
• Submitting bills to Medicare when Medicare is not the beneficiary’s primary insurer.
• Promises of “free” services, then ask for your Medicare number and bill to Medicare.
• Offers to waive Medicare deductibles or coinsurance.
