Adult Day Care—Provision of personal care for
dependent adults in a supervised, protective, congregate setting
during some portion of a 24-hour day.
Advocacy—Action
taken on behalf of an elder person to secure rights or benefits.
Assisted
Transportation—Provision of assistance, including
escort, to a person who has difficulties (physical or cognitive)
using regular vehicular transportation.

Caregiver
Support Program—Provision of information and
assistance, education, counseling, and funding for respite
and supportive services to persons aiding an adult aged 60+
years who needs help with at least 2 activities of daily living.
These services can also be accessed by a 60+ year old grandparent
caring for a child.
Case Management—Provision of assistance to facilitate access to or coordination of services to people with multiple or complex needs from several service providers.
Chore—Provision of services to help people handle household chores such as heavy cleaning, yard and walk maintenance.
Congregate Meal—Provision of a meal to an eligible client at a nutrition site, senior center or another congregate setting which complies with the Dietary Guidelines Allowances (RDA) as established by the Food and Nutrition Board of National Research Council of the National Academy of Sciences.
Counseling—Provision of services to advise and enable the older person to resolve problems or relieve temporary stresses via interview, discussion or lending a sympathetic ear.

Employment—Programs
to increase employment opportunities for older workers in
the general labor market as well as in special employment
programs.

Health
Promotion—Provision of activities designed
to prevent disease and promote health.
Health Screening/Well Elderly Clinics—Provision of standard examinations, procedures or tests to gather information about a client to determine need and/or eligibility for health services.
Holiday Meals—Provision of a meal to an eligible client’s place of residence on a holiday when regular congregate and home-delivered meals are not available. These meals must meet he same dietary standards of the regular meal program. Private fund-raising efforts support this program.
Home-Delivered Meal—Provision of a meal to an eligible client’s place of residence which complies with the Dietary Guidelines for Americans and serves a minimum of one-third of the current daily Recommended Daily Allowances (RDA) as established by the Food and Nutrition Board of the National Research Council of the National Academy of Sciences.
Homemaker—Provision of services to help people experiencing difficulty with activities of daily living such as preparing meals, shopping, managing money, using the telephone and light housekeeping.
Home Repair—Provision of services to improve or maintain residence, appliances, etc.

Medication Management—Provision of services that includes medication management, screening, and education.

to promote better health by providing accurate and culturally
sensitive nutrition, physical fitness, or health (as it relates
to nutrition) information and instruction to participants
and caregivers in a group or individual setting.

Restore Trust, now Senior Medicare Patrol —Provision of education to Medicare
beneficiaries to help reduce Medicare waste, fraud, and abuse.
Outreach—Provision
of interventions initiated by a provider to identify clients
and encourage them to use existing services.
