Case Management for the Frail Elderly
The Case Management Program coordinates community services that help participating older Iowans remain in their homes.
Under the program, an initial visit will be arranged in the home of the participant to ask questions, assess the situation, and understand individual needs. The interviewer will ask permission to discuss the information with a team of people from service agencies in order to coordinate the assistance needed.
One person on the team will be the Case Manager. The Case Manager will arrange for the services chosen by the participant and family members, and will continue to coordinate these services and act as the participant’s advocate.
There are no income guidelines and no cost to participate in the Case Management program. However, there may be fees for individual services. Medicare or Medicaid may cover the cost of some services or there may be a sliding fee scale. Some participants will be eligible for the Title XIX Elderly Waiver to cover the cost of services.
