Whether you have a simple question or complex situation, our aging experts will help guide you in the process of problem solving your specific issues. The following are specific programs designed to address your specific needs or an older adult you are advocating for.
Family Caregiver Program supports people who are caring for their parents, spouses and others who are 60+, as well as grandchildren. The program offers individual counseling, consultation, information, respite and general support services to enable caregivers to continue their roles.
Case Management serves persons over 60 who want to stay in their homes and live independently in the community. Staff coordinates home-based services on behalf of their clients.
Information & Assistance/ Benefits Specialist staff answers your questions regarding providers and services in your respective community. Staff will research and provide answers when you need them most.
Nutrition & Health Education Programs promote healthy eating through nutritious meals home delivered or at a congregate meal site.
Mental Health & Aging staff uses the Healthy IDEAS and PEARLS, evidence based training models. We also partner with Eyerly Ball.
Wellness & Safety programs regarding chronic diseases and fall prevention are offered throughout our entire service region. These programs empower seniors to be proactive partners in their health and well-being.
Iowa SMP (Senior Medicare Patrol) encourages seniors to prevent Medicare fraud by reviewing Medicare summary statements, report incorrect billing of services and protect personal identity information. SMP staff serves as advocates.
